Forms for Adoption

Forms to Ask the Court to Approve an Adoption:

If you want an amended Birth Certificate with the names of the adoptive parents on it then get a copy of the following form and fill out (in black ink only ) Part II:

If this is going to be an “open adoption” -- allowing continuing contact between the birth relatives, including birth parents and the child if the court agrees it is in the best interest of the child -- then get and fill out:

  • Contact after Adoption Agreement (Form ADOPT-310)

FOR DIFFERENT TYPES OF ADOPTIONS

Forms for an Agency Adoption:

In addition to ADOPT-200, ADOPT-210 and ADOPT-215,
you will need the Agency report an:
  1. Consent to Adoption by Parent(s) (In California)
    (Social Services Form AD 1A)
    OR (Outside California, Social Services Form AD 1C)
  2. Adoption Petition - Consent and Joinder (Social Services Form AD 824)
  3. Adoption Expenses (Judicial Council Form ADOPT-230)



Forms for Adoption of a Child of Indian Ancestry:

In addition to ADOPT-200, ADOPT-210 and ADOPT-215,
you will need an Agency report and:
  1. Adoption of Indian Child (Form ADOPT-220)
  2. Parent of Indian Child Agrees to End Parental Rights (Form ADOPT-225)
  3. Adoption Expenses (Form ADOPT-230)



Forms for an International Adoption:

In addition to ADOPT-200, ADOPT-210 and ADOPT-215,
you will need:
  1. Adoption Petition - Consent and Joinder (Social Services Form AD 824)
  2. Adoption Expenses (Judicial Council Form ADOPT-230)

NOTE: An agency report is not required to set a hearing on International adoptions. Granting an international adoption is subject to judicial determination when there is no report.



Forms for a Step Parent or Domestic Partner Adoption:

In addition to ADOPT-200, ADOPT-210 and ADOPT-215,
you will need an Agency report and

Consent to Adoption by Parent(s) (In California) (Social Services Form AD 1A)
OR (Outside California, Social Services Form AD 1C)



Forms for Adoption of an Adult:

No other forms are needed in addition to ADOPT-200, ADOPT-210 and ADOPT-215

  • Request for Accommodations by Persons with Disabilities and Response
    (Form MC-410)
